The form above shows the following:

  1. Name of the Teacher
  2. Room Number
  3. Previous Week's Date.
  4. Student Name
  5. ATTN = Student Identification Number
  6. B= Language Classification of the Student. L signifies an English Language Learner. E signifies an English Mainstream Student.
  7. C= Language of the correspondence sent home. S is for Spanish, while E is for English.
  8. Student's phone number.
