Cums

Permanent Record Card

Record grades and NUMBER OF DAYS ABSENT, (this is very important) on the top line of the grade level. This is vital information needed for the ACT program.

Record the following:

  • Enter and leave dates
  • Teacher's Name
  • School Name

Use LETTER grades on perm card for ALL grade levels. A -F or V-S-N is acceptable.
